Help me clean up my clover bootloader

I have four entries in my clover bootloader

• Boot Windows from (doesn't work)

• Boot Windows from (doesn't work)

• Boot Microsoft EFI boot menu from EFI

• Boot OS X

I have two entries in my UEFI shell

• Clover

• Windows Boot Manager

I can't figure out where the two errant "Boot windows from" entries are coming from. Everything I see points towards there only being two boot entries
